Describe precontemplation
patient not considering change, physicians goal is to increase awareness of why change should be considered
Describe contemplation
patient considering possibility of making changes, but uncertain, physicians goal is to encourage making a change
Describe preparation
patient committed to making change in near future but considering what to do, physicians goal is to clarify goals and strategies
Describe action
patient actively making changes, physicians goal is to reinforce importance of remain on track with changes
Describe maintenance
patient has made change, physician’s goal is to reinforce and support changes
