What should be considered when a patient comes for a well visit?
Get a thorough history, social history, evaluate any risk factors based on lifestyle and family history
What is a USPSTF recommendation?
recommendations of certain clinical preventative services put together by experts who make evidence based recommendations
Importance of evidence based medicine
allows patient to have best outcomes and provides patient most effective care based on evidence available
What is included in preventative services?
screening, immunizations, general health guidance, counseling to reduce risky behaviors
What is primary prevention?
intervention to prevent disease, includes vaccine, diet counseling, tobacco counseling, etc.
What is secondary prevention?
screening test for dz early while patient may still be asymptomatic or before dz onset (BP checks, labs, mammogram)
What is tertiary prevention
Clinical intervention that prevents dz progression or reduces complications (BP meds, chemo, etc.)
Grade A or B practice suggestion
provide service
Grade C practice suggestion
offer or provide service depending on circumstances
Grade D practice suggestion
Discourage use of service
Importance of well visits
used to treat current medical conditions and provide preventative care to decrease future health deterioration
Providing preventative services should be based on what three factors?
pt age, gender, risk factors
Harms of screening
cost, anxiety, decreased benefits based on patient age or comorbidities
Is there a guideline for checkups/well visits?
No!
Clinical rule of thumb to consider checkups
Every 3 ys in pts <49 without chronic medical issues; Every year for pts > 50 without chronic medical issues
How often should diabetic patients receive a check up?
Every 3 mo for A1C monitoring
How often should patients with HTN receive a check up?
Every 3-6 mo depending on control of BP
When to screen for Colon CA?
every 10 yrs at ages 50-75
When to screen for lung CA?
CT 55-74 yr old with at least 30 py history
When to screen for cervical CA?
Pap smear 21-65, frequency dependent on risk factors
When to screen for breast CA?
mammogram 50-65
What should be screened to asses for CVS risk?
diet, smoking, physical activity, hypertension, dyslipidemia, DM, obesity
How often should a person get an influenza vaccine?
every year
Frequency of Td/Tdap
every 10 years
