MEDICAL CAUSES OF PSYCHOSIS
1) CNS (cerebravascular disease, MS, neoplasm, huntington, parkinson, alzheimers, prion, tertiary syphilis, temporal lobe epilepsy, encephalitis, prion disease, neurosarcoidosis, AIDS) 2) endocrinopathies (addison/cushing, hypopituitarism, hypo/hyperthyroid, hypo/hypercalcemia) 3) NUTRITIONAL (b12, thiamine, folate) 4) connective tissue disease, temporal arteritis, porphyria, SLE)
Substances that cause psychosis
Steroids, antiparkinson agents, anticonvulsants, antihistamines, anticholinergics, some antihypertensives (beta blockers, digitalis, methylphenidate and fluoroquinolones).
Drugs illicit that cause psychosis
Alcohol, cocaine, benzos, hallucinogens,MJ, barbiturates, PCP
Medication/substance induced mania
Sympathomimetics, antidepressants, corticosteroids, levodopa, dopamine, bronchodilators
Medical causes of manic episode
hyperthyroidism, HIV, neoplasms, neurological disorders (temporal lobe seizures, multiple sclerosis)
Medication/substance induced depression
antipsychotics, anticonvulsants, sedative-hypnotics, ethonol, antihypertensives, barbs, corticosteroids, levodopa, diuretics, sulfonamides, withdrawal from psychostimulants
Medical causes of depressive episode
Endocrinopathies (Cushing syndrome, DM, Addison disease, hypolygcemia, hyper/hypothyroidism, hyper/hypocalcemia), cerebrovascular disease (stroke, MI), parkinsons, viral illness (mono), carcinoid syndrome, cancer (esp lyphoma and pancreatic cancer), collage vascular disease (systemic LE)
Medical causes of anxiety
Cardiovascular disease, anemia, low B12, pheochromocytoma, hyperthyroidism, hypoxia, neurological abnormalities (epilepsy, brain tumor, MS, cerebrovascular, etc), hypoglycemia
substance induced anxiety
Caffeine intake and withdrawal, theophylline toxicity, amphetamines, alcohol and sedative withdrawal, mercury or arsenic toxicity, penicillin, sulfonamides, sympathomimetics, antidepressants
Panic attacks are a/w which conditions
MV prolapse, asthma, PE, angina, anaphylaxis
differential dx of panic attack/disorder- medical
Medical: CHF, angina, MI, thyrotoxicosis, temporal lobe epilepsy, MS, pheochromocytoma, carcinoid syndrome, COPD
differential dx of panic attack/disorder- medications
amhetamine, caffine, nicotine, cocaine, hallucinogen intoxication, alcohol or opiate withdrawal
Psych disorders commonly a/w panic disorder
Depressive disorder, phobic disorder, OCD, PTSD
