Define Mania
A pathological, inappropriate elevated mood associated with grandiosity, disinhibition & loss of judgement
Similar to stimulant drugs
What is hypomania?
A lesser form of mania that lasts several days
What are the symptoms/signs of a hypomanic episode?
- Mildly elevated mood
- Increased energy & sense of wellbeing
- Sociability, overfamiliarity, sexual energy & decreased sleep
- Reduced conc
- New interests & mild overspending
- Irritability
- No psychosis
Hypomania shouldn’t cause severe disruption of work or social rejection
What are the symptoms & signs of mania?
Same as hypomania but worse +:
- Disinhibition
- Grandiosity
- Altered senses
- Extravagant spending
- Frequent irritability
- Pressure of speech
- Flight of Ideas
This tends to last ~1wk and is severe enough to completely disrupt work and socialising
DDX for mania?
Drugs & alcohol
SLE
Stroke, MS, tumour, epilepsy, AIDS & neurosyphilis
Cushing’s or Hyperthyroid
Mixed Affective state
Schizo(phrenia)(affective disorder)
Cyclothymia
ADHD
What tools do we use to assess mania?
SCID & SCAN
YMRS - Young Mania Rating Scale (mostly for monitoring how symptoms change)
How can we treat Mania?
- Antipsychotics
- Mood Stabilizers (anticonvulsants & lithium)
- ECT
What is BPD?
Bipolar disorder is a swinging of mood between depressive episodes and manic episodes
How do we diagnose BPD?
2+ episodes of depression &(hypo)mania
Mania is BPD, it cannot be diagnosed as just Mania.
When does BPD/mania tend to onset?
Earlier than depression, avg at 21.
Early onset i.e. <20 is commoner with a FH
Are mood disorders genetic?
Not in the sense that we can see the genes
But they are more common in 1st degree relatives
Do environmental factors have an affect on BPD’s epidemiology?
No change with income, occupation or education
Or gender fyi
How does psychiatry define mania?
no clear and convenient division:
- persistence of symptoms
- pervasiveness of symptoms
- degree of impairment
- presence of specific symptoms or signs
Outcome
60% recover at 10 weeks
5% fail to recover
10% die by suicide
90% have further episodes
How long does an episode tend to last?
1-3 months
