DSM-4 Criteria for Dx Major Depression
Need 5/8 symptoms for at least 2-weeks. Symptoms must include Depressed Mood or Anhedonia (interest). No history of hypomanic or manic. -SIG E CAPS Sleep Interest Guilt Energy Concentration Appetite Psychomotor agitation or retardation Suicidal Ideations Note: Can have major depression with psychotic features
DSM-4 Criteria for Dx Manic Episode
Elevated, expansive or irritable mood for AT LEAST 1 WEEK, and needs 3/7: DIGFAST Distractability Insomnia Grandiosity Flight of Ideas Activity/agitation Speech (pressured) Thoughtlessness

Mixed Episode
Meet criteria for both manic episode (3/7 DIGFAST) and major depressive episode (5/8 SIGECAPS). Must have symptoms everyday for AT LEAST 1 WEEK.
Hypomanic episdoe
elevated, expansive, or irritable mood with 3/7 Hypomanic symptoms:
- lasts at least 4 days
- no social/functional impairment
- no hospitalization needed
- no psychotic features
Age of onset for Major Depressive Disorder (MDD)
onset at any age
average = 40yo
triad for seasonal affective disorder
1) irritability
2) carbohydrate craving
3) hypersomnia
Seasonal affective d/o = subtype of MDD where episodes happen only during winter months.
Bipolar I d/o
- “manic depression” = symptoms of mania and depression
- Dx = only need 1 manic or mixed episode
- may have psychotic episodes
Bipolar I d/o
- “recurrent major depressive episodes w/ hypomania”
- Need 1+ major depressive episodes and at least 1 hypomanic episode
Side effects of lithium
- tremor
- seizures
- nephrogenic DI –> polyuria/polydypsia
- Heart Block –> Arrhythmia
- goiter/hypothyroid
- weight gain
- leukocytosis (benign)
- GI disturbance
- Fatigue
- coma
- alopecia
- metallic taste
Dysthymic Disorder Symptoms
CHASES
- Concentration or can’t make decisions
- Hopeless
- Appetite (+/-)
- Sleep (+/-)
- Energy low
- Self-esteem low
Dysthymic D/O rule of D’s
2 years Depression
2 listed Criteria
never asymptomatic for >2mo
NOTE: dysthymia can NEVER have psychotic features
DSM-4 Dysthymic d/o diagnosis
- Depressed mood most of the time for 2 years (1yr in children)
- At least 2 symptoms from CHASES
- During 2 years period: haven’t been w/o symptoms for more than 2mo + no depressive episodes
Cyclothymic d/o
- hypomanic symptoms with depressive symptoms for 2 years
- never symptom free for >2mo
- no Hx of MDD or manic episode
note: may occur w/ borderline PD, onset 15-25
