What constitutes mild, moderate and severe depressions respectively?
Mild = 2 core + 2 others, able to function Moderate = 2 core + 3 or 4 others Severe = 3 core + 4+ others, or 2 core + psychosis
What are the features of atypical depression?
Variable mood Overeating Oversleeping Extreme fatigue Pronounced anxiety
If someone presents with depressive symptoms, what conditions should you consider?
Depression 2ndary to: - Neuro disorders - Endocrine - Metabolic - Anaemia - Medication induced - Substance misuse Other psych conditions: - Bipolar - Anxiety - Schizoaffective - PD
Outline the management for depression based on the NICE guidelines
Mild = no antidepressants, self-help or group CBT, reassess in 2/52 Moderate = either antidepressants or high intensity CBT/IPT Severe = antidepressants and high intensity CBT/IPT, +/- 2nd antidepressant or ECT
How long should someone remain on antidepressants for?
If 1st episode, 6 months
If 2nd+, 2 years
What is the prevalence of bipolar disorder?
M:F ratio?
0.3 - 1.5%

What are the key features of bipolar disorder?
Recurrent episodes of altered mood and activity - either depressive, manic, hypomanic or mixed
What constitutes ICD-10 diagnosis?
2 episodes with resolution in between, one of which must have been manic or hypomanic
What is the core symptom of mania?
List some other symptoms. How many of these are needed for true mania?
Core = elevated mood for 1 week+ Others - 3+ = - Increased energy/activity - Grandiosity - Pressure of speech - Flight of ideas - Increased libido - Social disinhibition - Psychosis
What symptoms constitute hypomania?
How many and for how long?
3+ for 4+ days: Mildly elevated mood Increased energy Increased self-esteem Sociability Increased libido Decreased need for sleep
