MSK Requirements for CDL
1) No loss of foot, leg, hand, arm, OR has a SPE certificate
2) Has no impairment of
-Hand, finger interfering with prehension or power grasp
-Arm, foot, leg that interferes with ability to operate CMV, OR has SPE
3) Has no rheumatic, arthritic, neurologic, orthopedic, nuero/muscular or vascular disease that interferes with ability to safely operate CMV
With regards to MSK conditions, Examiners should consider 4 Main Things:
1) Nature and severity of condition (sensory or strength loss)
2) Degree of limitation (ROM for example)
3) Likelihood of progressive limitation
4) Likelihood of sudden incapacitation
What Is the SPE Certificate For?
FIXED deficits only. Not progressive issues (like Multiple Sclerosis, for example).
Where does on apply for an SPE Certificate?
FMCSA website … renewal period is every 2 years
What Type of Physician Can Do The SPE Eval?
Physiatrist and/or Orthopedic Surgeons (Board Eligible/Board Certified)
FMCSA recs going to a rehab facility
Can Canadian Drivers with Limb Impairments Drive without SPE?
Yes - essentially their standards meet ours or whatever
Autonomic Neuropathy Recs
Don’t qualify until etiology is clear, and treatment is proven to be safe, effective and stable.
2 year cert
Do not certify for sxs of orthostatic hypotension, resting tachycardia, or any other organ neuropathy that interferes with driving.
Neuromuscular Disease Rec
Same as everything (don’t qualify unless etiology clear and treatment works and is stable).
2 year cert
DQ for myotonia, Isaac’s syndrome, Stiff-Man Syndrome
Does Multiple Sclerosis DQ Drivers Automatically?
No not necessarily - need to have a great exam though
Nephropathy Recs
Don’t Certify Until Etiology confirmed and treatment safe/stable
2 year cert
Is there a recommended waiting period for people with nephropathy ?
No i guess not
Is Proteinuria or Hematuria Disqualifying?
No, this is up to Examiner based on amount of protein - NEED TO RECOMMEND FOLLOW UP
Dialysis Recs?
DQ FO SHO
Hernia Recs
Etiology needs to be known, and driver should be treated/safe/stable
Does Carpal Tunnel Require SPE?
NO. It’s not a fixed deficit. It can be cured, or if left untreated it can progress and get worse.
