Disqualifying Vision Findings
Less than 20/40 in each eye separately and/or both eyes
Monocular vision is disqualifying (possible exemption as long as monocular contact lenses aren’t used)
Required Vision Tests
Central Acuity, Peripheral Vision, Color Vision (only red/green/amber)
Near vision / depth perception NOT required.
When is Audiometry Required?
When Driver fails Whisper Test (office audiometry can’t test persons with hearing aids)
Meniere’s Disease
Disqualification is Recommended
Exemptions Are Made/Submitted to FMCSA For What Conditions?
Hearing & Seizure (no vision/no diabetes anymore)
Vertigo
Uncontrolled Vertigo Recommended To be Disqualifying
Certification is recommended for BPPV and Acute/Chronic Peripheral Vestibulopathy (IF at least 2 months symptom free)
Labyrinthine Fistula & Nonfunctioning Labyrinth
Recommend DQ’d
Diabetics on Oral Diabetic Meds Certification Time
2 years max… 1 year is the recommendation BUT should be documented if not done
What eye conditions must a med examiner ask about, and refer to specialist if indicated?
Retinopathy, cataracts, glaucoma, macular degeneration, and aphakia.
Who Can Perform Eye Exam?
Med Examiner, Optometrist or Ophthalmologist
Hypoglycemia Definition and Certification Recs
No more than 1 severe episode in 12 months, or 2+ in last 5 years
Severe Definition:
- Seizure, LOC, Coma or Needs Assistance From others
Diabetes Sxs That Should Be DQ’d
Loss of position sensation, loss of pedal sensation, resting tachycardia, orthostatic hypotension, neuropathy or proliferative retinopathy.
Can Drivers on Insulin Pass an Examination?
Yes. Provided they pass all the requirements and have a mandatory form filled out by their “Treating Clinician”
How often do drivers on Insulin need to be examined?
YEARLY! No exceptions (could be more frequent, of course)
MCSA-5870
This is the form that is filled out by a driver’s “Treating Clinician” for their insulin / diabetes.
This form must be completed within 45 days of any CDME, or a new one will have to be completed.
What happens if a Treating Clinician fills out an MCSA-5870 but doesn’t have 3 months preceding glucose monitoring logs?
Driver can be certified for up to 3 months, and during that time the logs must be kept and a NEW MCSA-5870 be filled out by TC
Blood Glucose Self Monitoring Reqs
Drivers must record 3 months of BG levels as instructed by TC. These records must be electronically kept, downloaded, and printed by glucometer. (Records dates & times of readings).
CAN’T Manually Enter on paper!
Results are turned into TC when the MCSA-5870 is filled out
What happens if a driver on insulin experiences severe hypoglycemic episode?
Unlike non insulin treated Diabetic drivers, who can’t have 1 episode within 12 months or 2+ within 5 years…
Drivers on insulin are DQ’d until TC has addressed this and feels confident things are great.
Incretin Mimetic Therapy, Is It Considered Insulin?
NO!
Recommend getting letter from TC on specifics (efficacy, toleration, fu frequency/monitoring)
