Module 4 - midterm Flashcards
is purposefully planned action designed with the intent of changing nutrition-related behaviour, risk, factor, environmental condition, or aspect of health status and is a critical component of the NCP.
Nutrition intervention
The primary purpose of nutrition intervention
to resolve and /or improve the nutrition diagnosis or nutrition problem (eliminating or altering nutrition etiology or relieving the signs/symptoms of the nutrition problem)
3 Steps in nutrition intervention
► Selecting and Planning: RDN prioritizes the nutrition diagnosis(es) based on the severity of the problem, safety, and the patient needs or population needs
► Implementing: the action phase during which the RDN carries out and communicates the plan of care to all relevant parties
- Meals and snacks
- Supplements
- Enteral and parenteral - Nutrition
- Feeding assistance
- Feeding environment
- Nutrition-related -medication management
Food and/or nutrient delivery
Nutrition Intervention Domains
Food and/or nutrient delivery
Nutrition education
Nutrition counseling
Coordination of nutrition
care by a nutrition professional
Population based nutrition action
In-depth collaboration between RND and client that may include advanced counseling techniques such as CBT and Motivational Interviewing
Nutrition counseling
- Brief diet instruction in hospital at bed side
- Out patient instruction
Nutrition education
Collaboration and referral of nutrition care
Discharge and transfer of nutrition care in new
setting or provider
Coordination of nutrition care by a nutrition professional
Intervention for the improvement ofnutritional health of the population
Population based nutrition action
refers to the patient/client’s individual recommended dietary intake of energy and/or selected foods (2) and/or nutrients based on current reference standards dietary guidelines and the patient/client’s health and nutrition diagnosis
Nutrition Prescription
is any diet altered to include or exclude certain components, such as calories, fat, vitamins and minerals and are typically modified for therapeutic reasons, including treatment of high blood pressure, low body
modified diet
Problem Intervention Planning Model
Problem/Etiology
(Nutrition Diagnosis)
Goal/Expected
Outcome
Plan/Intervention
Evidenced based
Rationale
are those problems identified from the assessment section and written in PES format
Nutrition diagnoses
represents what the
problem will look like
when it is resolved (the
ideal)
Goal
is what to do to reach the goal (these are the plan component). Usually target the etiology of the ND problem
Intervention
e is the “why” of
the plan; it must be
“Evidence-Based”
Rationale
is important in building a trusting relationship between an RND and the patient/client/
population from the first meeting, usually the nutrition assessment of the NCP.
Communication
According to De Negri and colleagues the following are the result of effective communication in health care:
- The patient discloses enough information about the illness to lead to an accurate diagnosis.
- The provider, in consultation with the patient, selects a medically appropriate treatment acceptable to the patient.
- The patient understands his/her condition and the prescribed treatment.
- The provider and patient/client establish a positive rapport
- The patient/client and provider are both committed to fulfilling their responsibilities during treatment and follow-up care.
a supportive climate is established, where one person is allowed to speak and the other listens, attending to the message rather than his/her own internal
thoughts and feelings
Verbal communication
Types of Communications
Verbal communication
Nonverbal communication
communicate our feelings toward others and are critical to
patient/RND Relationships.
Nonverbal communication
is the process of receiving, constructing meaning from, and responding to verbal and nonverbal messages.
Listening
is a broader term that attempts to encompass the multiple dimensions of an individual or group.
