identification and formulating a specific nutrition problem(s) that can be resolved or improved through treatment/intervention by a nutritionist dietitian.
Nutrition diagnosis
is the process of determining which disease or condition explains a person’s symptoms and signs. It is most often referred to as diagnosis with the medical context being implicit.
Medical diagnosis
Excessive carbohydrate intake related to visits to eating sweets and sugary desserts as evidenced by diet history and high blood glucose level
Diabetes
Undesirable food choices related to history of anorexia nervosa and self-limiting behavior as evidenced by diet history and weight loss of 5 lbs
Anorexia nervosa
Altered gastrointestinal function related to cirrhosis of the liver as evidence by steatorrhea and growth failure
Liver failure
Excessive energy intake related to lack of access to healthy food choices (fast food eating) as evidenced by diet history and BMI of 36.
Obesity
the patient’s/client’s nutrition concerns and needs which is stated in a nutrition diagnosis format
“PES” :
Problem
Etiology
Signs/Symptoms.
Describe alteration in the patient/client’s nutritional status, concerns or needs
Problem or Nutrition
Diagnosis Term
Cause/Contributing Risk
Factors
Etiology
Clusters of subjective and objective factors that provide evidence that a problem exists. Quantify the problem and describe the severity. They should be measurable and linked to the etiology by the word “as evidenced by
Signs and Symptoms
Too much or too little of a food or nutrient compared to actual or estimated needs
● Energy balance
● Oral or nutrition support intake
● Fluid intake
● Bioactive substance
● nutrients
Nutrition problems that relate to medical or physical conditions that are abnormal.
● Clinical
● Functional
● Biochemical
● Weight
Knowledge, attitudes, beliefs, physical, environmental, access to food or food safety
● Knowledge and beliefs
● Physical activity and function
● Food safety and access
It describes the problem, its root cause, and the assessment data that provides the evidence for the nutrition diagnosis.
PES
Five steps required in formulating nutritional diagnosis in the PES format for documentation in the medical record are as follows:
- List all nutritional concerns from the nutritional assessment
- Analyze and correlate nutrition concerns
- Select optimal nutrition diagnosis term
- Identify etiology, signs and symptoms
- Write the diagnosis in the PES format
Causes/contributing risk factors that may affect or be related to
Category
Dietary intake, availability of safe and healthy foods, water, supplies
Access
Attainment of nutrition-related goals
Behaviour
Truth-facts of nutrition-related information; feelings regarding the truth/facts
Belief and attitudes
Values, social norms and customs, religion, political views
Cultural
Understanding about food and nutrition, health, nutrition related information
Knowledge
Physical ability to perform specific tasks, may also be cognitive
function
Physical Function
Medical and health status that affects nutrition status
Physiologic/metabolic
Patient history
Social personal
Medical or surgical therapies and care that affect nutrition status
Treatment
PES statement should therefore be:
Clear and concise
Specific to the patient/client
Limited to a single problem
Accurately related to one etiology
Based on signs and symptoms from the assessment data
is the analysis of facts to form a judgment. The subject is complex, and several different definitions exist, which generally include the rational, skeptical, unbiased analysis, or
evaluation of factual evidence
Critical thinking
