A

A

Patient comfort and safety

A

Stored serum

A

Some institutions save an extra aliquot of serum in the accession area refrigerator for a period of time (7 days.) This sample is then available if repeat or further testing is required without the need to recollect from the patient.

Central inquiry

A

All inquiries can be routed through the accession department. All samples either will, or have passed through the area, so tracking specimens is facilitated.

Central reporting

A

Most results are entered into the lab computer system by the technologist completing the test, but in some labs, some results may be entered by trained, reliable individuals in the accessioning department.

Requisition

A

Is simply the formalization of a request for laboratory analysis to be performed on a patient specimen. Without a requisition it would be impossible to keep track of all requests in an orderly fashion.

A

A

A

Computer generated requisition

A

Allows for tests to be entered on a ward or unit. The request is received by the laboratory computer and a requisition is generated in the accession department. Labels and work lists can also be easily generated from the system.

A
  1. Name or code of hospital, 2. Date and time of request, 3. Patient category (inpatient/outpatient.) 4. Location of patients room, 5. Pertinent medical history, 6. Patients health care number, 7. Other insurance number/WCB number, 8. Priority of test (STAT.), 9. Time specifications of the tests to be collected, 10. Name of other physicians to receive the report, 11. Physician information - office location, 12. Hours PC (post consumption) when did the patient eat last.

Legal obligations

A
  1. All writing must be done with water resistant pen. Pencil is not allowed. 2. All corrections in laboratory required a single line through what is being omitted with initials, information clearly re-written. Scribbling and whiteout is not permitted. 3. All collections must be verified according to institutional protocol. The date, time and collectors identification are required.
