In the 2nd and 3rd trimester the amniotic fluid is produced primarily by what?
Kidneys and a small amount by the lungs
Any high GI obstruction or swallowing deficits causes what?
Polyhydramnios
Any bilateral GU obstruction/renal agenesis causes what?
Oligohydramnios
What is the maximum vertical pocket?
Deepest pocket that is free of umbilical cord or fetal parts
What is the AFI?
Largest pocket in each quadrant that is added together
What is a normal AFI?
5-25 cm
Maximum amniotic fluid is reached when?
22 weeks, maintained until 34 weeks when it slowly diminishes
What size of pocket is required for fetal breathing?
1 cm
Polyhydramnios would mean what size of pocket and AFI?
> 8cm pocket and >25cm AFI
What is the most common cause of polyhydramnios?
Diabetes mellitus
What is performed to reduce the fluid with polyhydramnios?
Amniocentesis
What are complications of amniocentesis? (3)
- PROM (premature rupture of membranes)
- Preterm delivery
- Infection/amnionitis
What is the purpose of indomethacin?
Prostaglandin inhibitor which decreases the fetal urine output in order to treat polyhydramnios
