What is PQRST?

A

P- palliative/provocative factors= what makes it better or worse?
Q- quality of pain= what kind of pain are you experiencing? Pain score?
R- region and radiation of pain = location? does it radiate?
S- severity of pain = rate 0-10, rate worst, best and now for the pain. Mild, moderate, severe?
T- temporal aspect of pain = is the pain better/worse at any particular time? Is it continuous or intermittent?

A
Way of assessing the wound.
T- tissue, non variable or deficient
I- infection or inflammation
M - Moisture imbalance
E- edge of wound, non advancing or undermining

What is the ABCDE rule for melanomas?

A

It is used to determine changes in the most common types of melanomas.
A- asymmetry = parts of the lesion differ in shape
B- border irregular = edges of lesion are irregular in appearance.
C- colour changes = pigmentation consists of red, white or blue colours mixed with black, tan or brown.
D- diameter = usually larger than 6mm.
E- elevation/evolving- raised above the surface and has an uneven surface. Looks different from the rest or changing in size, shape and colour.

What does the acronym R.I.C.E stand for and what it is used for?

A

R.I.C.E is commonly used for sprains and strains.
R=rest= helps limit movement that can worsen the injury
I=ice= applied to the area, reduces swelling and helps stop any bleeding
C=compression= apply pressure to the area or bandage the area to help stop bleeding and swelling
E=elevation= raising the injured area above the rest of the body (chest height) as a way to reduce swelling.

It is a quick GCS.

V=verbal
P=painful stimuli

A
FAST is used to tell if someone is having a stroke.
F=face-check the face for droopage
A=arms-can they lift both arms?
S=speech-is their speech slurred?
T=time-time is of the essence.
