Lower Trapezius
PRONE
Scapular adduction and depression
- STAND on contralateral side
- Shoulder abducted to 130 degrees and externally rotated; point thumb to ceiling, face away from side tested
- STABILIZE on other scapula
- PALPATE as pt goes through full ROM
- PRESS on ACROMION PROCESS in line with lower trap muscle fibers
- Note: Avoid pressing down on humerus, you are looking at GH muscles only
Middle trap
PRONE
Scapular adduction
- STAND on contralateral side
- Shoulder externally rotated; elbow bent at 90 degrees, palm down, face away from side tested
- STABILIZE on other scapula
- PALPATE as pt goes through full ROM
- PRESS on acromion process in line with middle trap muscle fibers
- Note: Avoid pressing down on humerus, you are looking at GH muscles only
Rhomboid
PRONE
Scapular adduction and downward rotation
- STAND on contralateral side
- Back of hand on low back region, shoulder internally rotated, head away from side being tested
- STABILIZE ipsilateral scapula
- PALPATE as pt goes through full ROM
- PRESS on medial border of scapula towards acromion process
- Note: Avoid pressing down on humerus, you are looking at GH muscles only
Shoulder extension
PRONE Latissimus dorsi Teres Major - STAND on contralateral side - Shoulder barely abducted, palm up - STABILIZE medial border of same scapula - PALPATE as pt goes through full ROM - PRESS down above the elbow (push into flexion and abduction)
Posterior Deltoid
PRONE Horizontal abduction - Have patient hang arm off table so elbow is bent at 90 degrees - STABILIZE opposite scapula - PALPATE as pt goes through full ROM - PRESS down on back of arm
Shoulder external rotators
PRONE Infraspinatus Teres Minor - Place towel roll under arm - Hang arm off table so elbow is 90 degrees - STABILIZE scapula on same side -PALPATE through full ROM - PRESS down proximal to wrist
Shoulder internal rotators
PRONE Subscapularis Pec Major Latissimus Dorsi - Place towel roll under arm - Hang arm off table so elbow is 90 degrees - STABILIZE scapula on same side - PALPATE through full ROM - PRESS down proximal to wrist
Cervical Extension
PRONE - With arm at 90 degrees, elbow bent at 90 degrees - STABILIZE on upper thoracic spine - PALPATE as pt lifts head up - PRESS against back of head
Pectoralis Major
- Clavicular fibers
SUPINE
Horizontal adductor
- Hold arm out, elbow bent at 90 degrees
- STABILIZE under same scapula
- PALPATE full ROM as pt brings arm across body “shield”
- PULL arm towards you
Pectoralis major
- Sternal fibers
SUPINE - Abduct arm out at 130 degrees - STABILIZE diagonal ASIS - PALPATE full ROM as pt puts sword into pocket - PULL arm towards you
Elbow extension
SUPINE o Triceps Brachii Elbow pointing to ceiling, hand folded down Patient extends arm part of the way Stabilize on shoulder of same side Press arm back down into flexion
Serratus anterior
SUPINE
o Scapular abduction and upward rotation
o Arm is in front of patient
o Instruct patient to punch wall in front
o “Lock shoulder in place” so that you can push back on arm and affect scapular movement
o Stabilize under scapula
Cervical muscles
Flexion
SUPINE With arm at 90 degrees, elbow bent at 90 degrees Stabilize on sternum Lift chin to chest Press against forehead
Cervical muscles
Rotation
SUPINE
With arm at 90 degrees, elbow bent at 90 degrees
Palpate SCM
Instruct patient to turn head to other side
Stabilize on sternum
Press against temple further into rotation
Upper trapezius / levator scapula
Bilateral
SITTING o Bilateral scapular elevation o Stand behind patient o Patient is instructed to elevate shoulder toward head as far as possible o Press shoulders down out of shrug
