Patient-related and Surgery-related factors that could affect preoperative risk assessment
Emergency vs Urgent Procedure
if without coronary intervention, how many days should elapse before undergoing noncardiac surgery after MI.
≥60 days
Procedure-specific risk: surgical risk estimate according to type of surgery or intervention
Consideration in pt with Restrictive Cardiomyopathy undergoing noncardiac surgery
preload and HR dependence in CO
Consideration in pt with Hypertrophic Cardiomyopathy undergoing noncardiac surgery
avoid arterial vasodilator, volume loss or reduction in preload
for severe valvular stenosis (asymptomatic)
- avoid hypotension and tachycardia
- intraoperative and postoperative hemodynamic monitoring
*** for favorable valve, PMBC for severe MS
Arrhythmias that can increase operative risk
- symptomatic/associated with hemodynamic compromise PVC and NSVT or associated with ssignificant structural heart disease or inherited electrical disorders
- High grade AV block
Low risk vs High risk cut off based on RCRI
0-1 point - low risk
≥2 - high risk
Classification of functional capacity
