What type of drug is tamoxifen?
A selective oestrogen receptor modulator (it partially anatgonises and agonises oestrogen receptors)
What is tamoxifen used for?
To treat oestrogen receptor positive breast cancer
What are some of the side effects of tamoxifen?
Hot flushes (3%)
Increased DVT risk (1.2% per year)
Menstrual disturbances
Oesteoporosis
Should metformin be stopped for surgery?
In minor surgery, no
In major surgery, yes and commence variable rate insulin
Ciprofloxacin’s effect on warfarin?
It inhibits the P450 system causing an increase in INR
How do we treat hyperhidrosis?
Aluminium chloride (applied via roll-ons at night-time) Can eventually use botulinum toxin injections and topical glycopyrrolate if unresponsive
What are the adverse effects of methotrexate?
Mucositis Myelosuppression Pneumonitis Pulmonary fibrosis Liver fibrosis
Methotrexate and pregnancy
Stop at least 6 months before pregnancy for m/f
Methotrexate and trimeothoprim
Both affect folate, can cause marrow aplasia
Avoid
Methotrexate supplementation
Give with folic acid (5mg once weekly, 24 hours after methotrexate dose)
Which medication to treat TB can have adverse effects on vision?
Ethambutol potentially causes optic neuritis
Opioid misuse complications?
Viral infections secondary to sharing needles - HIV, Hep B and C
Bact. infection - Infective endocarditis, septic arthritis, septicaemia, necrotising fasciitis
VTE
How do we treat opioid overdose?
IV/IM naloxone
What can topical corticosteroids cause in patients with darker skin?
Patchy depigmentation
