Neuroleptic malignant syndrome
Neuroleptic malignant syndrome should be suspected in any patient given a neuroleptic drug such as butyrophenone (Haldol) who subsequently develops muscle rigidity, obtundation, tachycardia, and temperature elevations. Unlike malignant hyperthermia, this syndrome is centrally mediated. The proposed mechanism is an imbalance of central neurotransmitters, specifically dopamine. Other drugs commonly used in intensive care units linked to neuroleptic malignant syndrome include thioxanthenes and metoclopramide. Specific treatment recommendations include dantrolene and bromocriptine. Bromocriptine has been advocated because of its ability to increase central neurologic/dopaminergic tone.
