100 mcg of 1:10,000 epi, repeat if needed, then increase to 200 mcg if needed
50 mg iv diphenhydramine
5 mg/kg hydrocortisone

If persistent hypotension epi gtt

Malignant hyperthermia tx?

Stop all meds
Dantrolene 2.5 mg/kg every 5 minutes

TB Management?

Non productive cough, hilar LAD, recent travel
FNA enlarged lymph nodes, then core needle, then excisional bx if nondiagnostic.
Admit for isolation, PPD, Quantiferon gold test,
Report to department of public health
Screen close contacts for active TB w PPD and CXR and if negative do prophylactic rifampin for 3 months
RIPE: rifampin, INH, pyrazinimid, ethambutol. Monitor LFT and treat for 6 months
If worsening on RIPE consider drug resistant TB or superimposed bacterial infection
