What is Milnirone?
Milnirone is pulmonary vasodilator used in patients who have heart failure. It is a phosphodiesterase 3 inhibitor that works to increase the hearts contractility and decrease pulmonary vascular resistance.
Milnirone is a vasodilator that is used as short term treatment for life threatening heart failure.
When should Milnirone be administered?
For short term treatment of severe congenital heart failure unresponsive to conventional maintenance therapy, not immediately after myocardial infarction
Acute heart failure including low output states following heart surgery initially by intravenous injection
What is the usually prescribed does for Milnirone?
Initially 50 mcg/kg, given over 10 min followed (by intravenous infusion) 375- 750 nanograms/kg/minute usually given following surgery for up to 12 hours or in congestive heart failure for 48- 72 hours.
What are the contra- indications for Milnirone?
Severe hypovolaemia
What are the side effects of Milnirone?
Common
- Arrhythmia supra ventricular (increased risk in patients with pre-existing arrhythmias
- headache
- hypotension
Uncommon
- Angina
- chest pain
- hypokalaemia
- tremor
Rare
- anaphylactic shock
What needs to be monitored when on Milnirone?
- monitor blood pressure
- heart rate
- ECG
- central venous pressure
- fluid and electrolyte status
- renal function
- platelet count and hepatic enzymes
What are the directions for administration for Milnirone?
- avoid extravasation
- For intravenous injection, may be given undiluted or diluted before use
- For intravenous infusion give continuously in Glucose 5% or sodium chloride 0.9% dilute to a suggested concentration of 200 mcg/mL

What is the mode of action of ibuprofen?
- Ibuprofen is a NSAID and it works by blocking your body’s production of certain natural substances that cause inflammation. This effect helps to decrease swelling, pain and fever
What are the contra indication for ibuprofen?
- You shouldn’t take ibuprofen if you have a history of a strong, unpleasant reaction to aspirin or other NSAID.
- stomach ulcer or if you have had one in the past
- severe heart failure
What is the duration of action of ibuprofen?
- Duration of action of 400mg is at least 6 hours compared with 4-6 hours for ibuprofen 200mg or paracetamol
What should you assess before giving ibuprofen?
Monitor signs of GI bleeding Abdominal pain Vomiting blood Blood in stools Black stools
