Midterm OP - Special Test Recall Flashcards
GH Dislocation/Instability
Confirming & Ruling Out
CONFIRM: Crank/Apprehension Test (anterior), Sulcus Sign (inferior), or Push-Pull (posterior)
> can add Jobe Relocation test, verbalize Rockwood modification
RULE OUT: Active Compression of O’Brien (SLAP lesion)
Adhesive Capsulitis
CONFIRM: pROM (scapulohumeral rhythm, CPR ER > ABD > IR)
RULE OUT: Infraspinatus Test OR Whipple Test
Shoulder Impingement
CONFIRM: Hawkins-Kennedy
RULE OUT: Speed’s
Subacromial Bursitis
CONFIRM: Empty Can Test OR pROM (passive range of motion)
Painful Arc Test (pain between 60-120* - after 120* compression on supraspinatus and the subacromial bursa decreases
> this also rules out adhesive capsulitis
> pain at 170-180* may indicate AC lesion
RULE OUT: Paxinos Sign
> Speed’s & Empty Can Test would both elicit pain with subacromial bursitis, too!
Acromioclavicular Sprain/Injury
CONFIRM: Paxinos Sign
RULE OUT: Empty Can (supraspinatus tendinopathy)
Biceps tendinitis
CONFIRM: Speed’s Test
RULE OUT: Yergason’s Test OR Empty Can
Transverse humeral ligament sprain
CONFIRM: Yergason’s test
RULE OUT: Speed’s test
Rotator cuff strain
CONFIRM: Whipple Test
>contractile
RULE OUT: Hawkins-Kennedy
>passive
Supraspinatus tendonitis
CONFIRM: Empty Can Test
RULE OUT: Speed’s OR Paxinos Sign
SLAP Lesion/Labral Tear
CONFIRM: Active Compression of O’Brien
RULE OUT: Paxinos Sign
TOS
CONFIRM: Wright’s OR Halstead
RULE OUT: Spurling’s
>rule out cervical radiculopathy
Serratus Anterior pathology
CONFIRM: Punch Out Test
RULE OUT: Wright’s or Halstead
>since serratus anterior refers down the arm and into the hand
Medial epicondylitis
CONFIRM: Golfer’s elbow test (passive) then add active component to confirm
RULE OUT: Valgus instability test
Lateral epicondylitis
CONFIRM: Cozen’s or Mill’s test
RULE OUT: Varus instability test
Ulnar/radial lateral collateral ligament sprains
CONFIRM: Valgus or varus instability tests
RULE OUT: Cozen’s or Golfer’s elbow tests
Olecranon bursitis
CONFIRM: aROM
>end range of elbow flexion limited d/t pain
RULE OUT: ISE/palpation/history - RA? Gout or pseudogout, fracture, OR Elbow Flexion Test
Cubital Tunnel syndrome
CONFIRM: Elbow Flexion Test
RULE OUT: Halstead, Wright’s OR Spurling’s
> Rule out cervical radiculopathy
Dequervain’s
CONFIRM: Finklestein’s
RULE OUT: Axial Load Test of Thumb
Trigger finger
CONFIRM: rROM (may hear snapping if tendon catches while moving, pain)
RULE OUT: pROM to rule out Dupuytren’s
Dupuytren’s
CONFIRM: pROM
RULE OUT: Froment’s sign to rule out ulnar nerve pathology
>ulnar nerve lesion could cause hypothenar atrophy
FDP Rupture
CONFIRM: History/MOI, Sweater Finger Sign
RULE OUT: Pronator Teres Syndrome Test
Mallet finger
CONFIRM: aROM
RULE OUT: pROM to ensure no contracture on flexor side such as trigger finger - snapping of tendons
Ganglion cyst
CONFIRM: visual scan with wrist in flexion & palpation, pROM wrist extension
RULE OUT: refer to doctor
RA or OA
CONFIRM: Axial Load or Ligamentous Instability test
RULE OUT: Finklestein’s, if in the thumb
Gamekeeper’s/skiier’s thumb
CONFIRM: Thumb Ulnar Collateral Laxity (compare BL, 15* is normal, up to 30* is partial tear & 30-35*+ is a full tear)
RULE OUT: Axial Load Test OR Finklestein’s
Scaphoid fracture
CONFIRM: Axial Load test
RULE OUT: Finklestein’s
CTS
CONFIRM: Phalen’s or Carpal Compression Test
RULE OUT: Pronator teres syndrome test
TFCC Irritation
