MI Flashcards
1
Q
Acute STEMI- Eti
A
- Prolonged imbalance between supply and demand of O2 blood
- Total blockage of coronary perfusion
- Elderly, women may not experience chest pain
2
Q
Acute STEMI- Sx
A
Angina at rest, early morning
- Pain builds rapidly- substernal, neck, jaw
- Minimally responsie to nitroglycerin or morphine
- Diaphoresis, weakness, impending doom
- Cyanosis, decrease peripheral pulse
- Rales, murmurs
3
Q
Acute STEMI- Dx
A
- CK-MB & Troponin elevations
- ST segment elevation on EKG
- New Q waves
4
Q
Acute STEMI- Tx
A
- Immediate reperfusion within- Stenting 2 hrs
- thrombolysis within 3 hours
- Aspirin
- Nitroglycerin
5
Q
Angina Pectoris- Eti
A
- Ischemia due to atherosclerortic heart disease
- Metabolic disorders- hyperthyroid, anemia,
6
Q
Angina Pectoris- Sx
A
- Precipitated by activity, relief with rest- short duration
- tightness, squeezing, burning in chest- fist clench over mid chest
- Radiates to left shoulder, arms, jaw, back
- Relief with nitroglycerin
7
Q
Angina Pectoris- Dx
A
- Exercise EKG- downsloping ST segment
8
Q
Angina Pectoris- Tx
A
Nitroglycerin, long term beta blockers, platelet inhibitors, lifestyle modifications
9
Q
NSTEMI- Eti
A
- Non- ST segment elevation due to MI or ischmia
- Unstable supply of o2 blood due to block
- Partial thrombus/ lysis
10
Q
NSTEMI- Sx
A
- Substernal chest pain at rest or minimal exertion
- Possible to have no pain
- Dyspnea, nausea, diaphoresis, syncope
11
Q
NSTEMI- Dx
A
- CK-MB & Troponin I
- New Q waves, ST wave depression,
12
Q
NSTEMI- Tx
A
- Antiplatelet/ anticoagulant
- Nitroglycerin
- Beta-blockers
- Statins
13
Q
Unstable angina- Sx
A
Similar to NSTEMI- uncontrollable by patient
14
Q
Unstable angina- Dx
A
- Microchanges in cardiac biomarkers
- EKG changes, no biomarker changes- T wave inversion, ST depression
- If Troponin above lower limit = NSTEMI
15
Q
Unstable angina- Tx
A
- Aspirin, platelet inhibitors
- Beta blockers
- Nitroglycerin
NO THROMBOLYTICS - Cath
