-dysuria, burning, incomplete emptying
-SOB diff. w/out exertion
pp assessment
-vitals every 15min for 1st hour, every 30min for 2nd hr, evey 4 hours first 24 hrs, every 8 after 24 hrs
-know the normal and keep risk factors in mind!!!!
-slightly elevated temp in first 24 hrs
-B.U.B.B.L.E, EE
-emotional status, bonding/attachment- immediately after birth
comfort
-application of cold pack in 4th stage, first 24 hrs, 20min on/10min off, cover with clean cloth, reassess
-peri bottle for perineal hygiene, after 24hrs can take a sitz bath (hydrotherapy)
topical preparations
-hemorrhoid creams, witch hazel wipes, dermaflash spray
elimination assessment
-bowel sounds, return of BMs, hemorrhoids
-promote BM (fluids, ambulation, fiber)
-stool softeners, laxatives
bladder assessment
-distention, tenderness
-ask about voiding patterns, pain
-promote frequent voiding, increase fluids, measure 1st voids after delivery, catheterize
rest, activity, & exercise
-early ambulation, encourage rest, promote exercise
-encourage sleep when infant sleeps
-reduce outside activities
-eat balanced diet for healing/energy
-lift nothing heavier than infant
-limit driving and stairs
-teach kege’s, pelvic tilt, pvt
-no sex 2-4 weeks after birth
-use contraception
nutrition
-encourage all food groups, protein aids in tissue repair
-2-3L of fluids
-avoid weight reduction diet and harmful substances
