S: Male sex
A: Age 15-25 or >59
D: Depression or hopelessness (2 points)
P: previous suicidal attempt or psychiatric care
E: Excessive ethanol or drug use
R: Rational thinking lost (psychosis) (2 points)
S: Single, widowed, divorced
O: Organized or serious attempt (2 points)
N: No social support
S: Stated future intent for suicide (2 points)
Scoring
- low 0-5
- require consult 6-8
- admission >8
Discuss the requirements, boxes and procedure for a Form 1
Requirements
- been formally assessed by a physician in last 7 days
Box A past/present and future test
- threatening harm to self
- attmepting to cause bodily harm to self
- behaving violently towards another
- lacking competence for care
Box B Must fulfill all 5 criteria
- past test of patient received treatment for box A critieria
- patient clinically improved on treatment
- patient is incapable of consenting to treatment
- patient suffering from same disorder
- future test of Box A
Procedure
- police have 7 days to bring patient in for assessment
- Form 42 then given to patient
- psychiatric assessment over 72 hour
Discuss a Form 2
- can be obtained by any person and completed by justice of the peace
- must meet Box requirements
- police then bring in person to be assessed by psychiatrist
Discuss Section 17
- police officer bring in to health care facility for reasons on form 1
Discuss the voluntary/involuntary, pre-requisite and procedure for Form 3
Involuntary - completed by psychiatrist which allows for involuntary admission to hospital Prequisite - patient must be on form 1 - must be completed by different physician than that of form 1 - must meet same safety criteria Procedure - form 30 need to be given to patient - can be admitted for 2 weeks
Discuss the pre-requisite and procedure for form 4
- is certificate of renewal to keep patient involuntarily admitted
Pre-requisite - satisfy criteria in form 1
Procedure - filled first time then for 1 month, 2nd for 2 months, 3rd for 3 months
- form 30 must be given to patient each time
Discuss the indication and pre-requisite for a community treatment order
- requires patient to comply with outpatient treatment plan
Indication - used for patients with poor insight, compliance and frequent admissions
Pre-Requisite - consent given by patient or SDM
- have history of admission and meet Form 1 criteria
- failure to comply patient gets readmitted to hospita
Discuss the components of informed consent
Patient receives information on
- nature of treatment
- expected benefits
- risk and side effects
- alternative treatment course or action
- likely consequence of no treatment
List the criteria for a capable patient
- able to understand information that is relevant to making decision
- able to appreciate reasonably foreseeable consequences of lack of decision
- able to apply information to their own situation
List the criteria for incapacity
- if patient is labelled uncapable they have 2 days to be apply to consent and capacity board
- all non-emergency treatment must be delayed until decision or 2 days have passed
- patient provided form 33
List the rank order for substitute decision makers
- power of attorney
- spouse or partner
- parent or child
- sibling
- any other relative
- public guardian and trustee if no one else available, wanting or 2 SDMs make claim with equal rank
