A
Fever
Drowsiness
Septic shock
Rash
3
Q
Investigation
A
CSF
Blood cultures
Throat swab
Blood to detect bacterial DNA by PCR
4
Q
Meningococcal menigitis
A
presence of rash!
Neisseria mengitidis
5
Q
Complement is triggered by
A
- antibodies bound to bacteral components
2. bacterial sugars bound to an immune protein called mannose binding lectin
6
Q
Immune evasion/ survival factors
A
- can have 14 different polysaccharide capsules which prevents formation of complement attack complex
- GNA1870 binds factor H which downregulates complement
- Type 4 pili bind c4 binding protein which cleaves the c3 convertase
- releases blebs of LPS which acts as a decoy
- porA also binds C4BP
- avoids immune system by expressing host structures: diff LPS that has identical structure to human blood group antigen
7
Q
Management
A
- IV antibiotics (penicillin)
- resuscitate
- blood cultures when IV line sited
- transfer to hospital
- investigations
- pain relief, fluids, IV antibiotics
- droplet precautions
- prophylaxis to contacts
If viral,
- reassurance
- analgesia
- recover at home usually
8
Q
Drugs
A
- cefazolin, cephalexin (oral)
- cefuroxime, cefaclor (oral)
- ceftriaxone, ceftazidime
- cefepime, cefpirome
- ceftaroline
9
Q
Management of septic shock
A
- maintain organ function - IV fluids, oxygen etc
2. resolve infection - antibiotics, surgery, etc
