A

focal area of subcondral bone undergoes necrosis. As the necrotic bone is reabsorbed the cartilage loses its supporting structure and often allows a bony fragment to be displaced into the joint space.

lead to knee pain worse with activity, and can become stiff after periods of disuse. Common complaints include a sensation of the knee giving way and INABILITY TO FULLY EXTEND THE EXTREMITY. On exam internally rotating the tibia with the knee flexed to 90 followed by careful extension produces pain (Wilson sign)
