A

STEP 1:

  • Enquirers details and contact info
  • Patient details
  • Details about question, drug history, PMH
  • Ethical concerns and why took action you did

STEP 2:
- Resources used

STEP 3&4:

  • Document answer exactly how given
  • Cite refs if written answer
  • Document who fed back to and when
  • Keep correspondence copies
  • Keep details of those who checked answer
