Atracurium (Mechanism of action, onset, duration of action, receptors, normal dose, metabolism, excretion, usage, risks)

A

MOA: Nondepolarizing neuromuscular blocker,compete with Ach for active binding sites at postsynaptic nicotinic Ach Receptor (competitive antagonists)/ benzylisoquinolinium NMBA(preferred for renal/hepatic patients)
Associated with histamine release

ONSET: 3-5 minutes

DOA: Intermediate(30-50min)

NORMAL DOSE: 0.5 mg/kg, The maintenance dose of atracurium is 0.1 mg/kg, or an infusion at 10 to 20 mcg/kg/min, (f/b neuromuscular monitoring)

METABOLISM: nonspecific plasma esterase-mediated hydrolysis and a nonenzymatic, pH- and temperature-dependent degradation called Hofmann elimination. Laudanosine is the final product of Hofmann elimination,

EXCRETION: half life 21 min, hepatic/renal,no metabolite

USAGE: INTUBATION

RISK: Inhaled anesthetics, some ABTs, inhibit nAChRs and potentiate neuromuscular blockade with non-depolarizing NMBAs.
Patients on Lithium, hypermagnesemic, Hypocalcemic prolonged response to NMJ (depol and non depol)
Acidosis,Hypothermia prolong effects of NMDA
HYPERCALCEMIA: prolongs NDMA

ANTIDOTE: NEOGSTIGMINE /SUGAMMADEX
