INDICATIONS - Conversion on PSVT to Sinus Rhythym DOSE Adult- 6 mg, repeat 12 mg RIVP Pediatric- .1 mg/kg, repeat .2 mg/kg RIVP MAX 12 mg
Albuterol Sulfate
INDICATIONS
- Relief of bronchospasms
DOSE
Adult- 2.5 mg, one unit dose
Amiodarone
INDICATIONS
- V-fib/ Pulseless V-tach
DOSE
Adult-
V-Tach w/ pulse: 150 mg in 100 cc NS over 10 minutes
V-Tach/ Pulseless V-tach: 300 mg IV bolus; repeat 150 mg
Pediatric- 5 mg/kg IV/IO over 20-60 minutes
Aspirin
INDICATIONS
-New chest pain suggestive of AMI
DOSE
-324 mg PO
Atropine Sulfate
INDICATIONS - Hemodynamically significant bradycardia DOSE Adult- Brady- .5 mg IV; MAX 3 mg OD/ Nerve- 2 mg IV/IO Pediatric- Neonatal resus/Brady- .02 mg/kg IV/IO Organophosphate- <12 yo: .02-.05 mg/kg; >12: 2 mg IV/IO
Calcium Chloride
INDICATIONS
- known or suspected hyperkalemia, Ca++ blocker OD
DOSE
Adult- .5g-1g IV/IO
Dextrose
INDICATIONS - Hypoglycemia DOSE Adult- 25g IV/IO of 50% soln Pediatric- Neonatal resus: 5-10ml/kg of 10% soln Hypoglycemia: 2-4ml/kg of 25% soln
Diazepam (Valium)
INDICATIONS - Seizure, muscle relaxant DOSE Adult- Seizures: 5-10 mg IV/IO Sedation: 5-15 mg IV/IO Pediatric- Seizures: .5 mg/kg PR
Diltiazem (Cardizem)
INDICATIONS
- Rapid Ventricular rates w/ A-Fib/A-Flutter
DOSE
Adult- 20 mg (.25 mg/kg) over 2 minutes; repeat in 15 minutes at 20-25 mg
Diphenhydramine
INDICATIONS - Anaphylaxis/ Allergic rxn DOSE Adult- 25-50 mg IV/IM/IO Pediatric- 1-2 mg/kg Slow IV/IM/IO
Dopamine
INDICATIONS
- Hypotension, Shock, Low CO
DOSE
Drip, supplied as 200 mg/ml, 400 mg/5 ml syringe
Duo-Dote Auto-Injector
INDICATIONS -SLUDGEM DOSE Adult- 1 unit for mild symptoms, 3 units for severe symptoms Pediatric- Weight based by broslow tape

INDICATIONS
Asthma, Allergic rxn, Cardiac Arrest, Asystole
DOSE
Adult-
Asthma/COPD/Anaphylaxis: .3-.5 mg IM, .3-.5mg infusion (1:10,000) over 5 minutes
Symptomatic Bradycardia: Infusion 2-10 ug/min IV/IO
Cardiac Arrest: 1 mg (1:10,000) IV/IO
Pediatric-
Etomidate
INDICATIONS To induce anesthetic sedation for medical procedures DOSE Adult- .2-.4 mg/kg over 30-60 minutes for MFI .15 mg/kg for sedation
Fentanyl Citrate
INDICATIONS Severe pain and anesthesia DOSE Adult- 1 mcg/kg slow IV to maximum 100 mcg/dose May be repeated to max individual dose of 200mcg
Furosemide
INDICATIONS Edema, CHF DOSE Adult- .5-1.0 mg/kg over 1 to 2 minutes, if no response double dose Pediatric- 1 mg/kg IV; MAX 20 mg
Glucagon
INDICATIONS AMS where hypoglycemia is suspected DOSE Adult- .5-1.0 mg IM or 2 mg IN Pediatric- .5-1.0 mg/kg IM or 2 mg IN <20kg; 20-30 ug/kg IM/IN
Haloperidol
INDICATIONS Adult Behavioral Emergency, Agitated, aggressive pt's Dose Adult 2-5 mg IV/IM
Hydrocortisone Sodium Succinate
INDICATIONS Adrenal Insufficiency DOSE Adult- 4 mg/kg SIVB/IM Pediatric- 2 mg/kg bolus IV/IM; Not to exceed 100 mg
Hydroxocobalamin
INDICATIONS Known or suspected Cyanide Poisoning DOSE Adult- IVP 5g/200ml NS in 15 minutes (15 ml/min) Max total dose of 10g Pediatric- 70 mg/kg over 15 minutes
Ipratropium Bromide
INDICATIONS Asthma, Bronchospasm, COPD DOSE Adult- unit dose Pediatric- .5 mg
Ketrolac
INDICATIONS
Moderate to severe pain relief
DOSE
Adult- 30 mg IV over 15 seconds, 60 mg IM
Lidocaine
INDICATIONS

Pre-intubation w/ suspected ICP: 1-1.5 mg/kg IV
MAX IND. dose 100 mg
Topical Anesthetic: 20-40 mg 2% at the injection site
Lorazepam (Ativan)
INDICATIONS Seizure, Muscle Relaxant DOSE Adult- MFI/RSI: 1-2 mg SIVP; may repeat to total of 4 mg after 5 min. MAX 8mg total Cardioversion: 2-4 mg IV/IM; may be repeated once, MAX 8 mg total Seizure: 2-4 mg IV/IM; MAX 8 mg Pediatric- .05-.2 mg IV/IM/IO; MAX .2mg/kg
