What is clozapine metabolized by?
Cytochrome P450 enzyme aka CYP1A2.
This decreases the serum level of other drugs that are substrates of that enzyme.
This can cause subtherapeutic drug level
Enzyme inducer definition
If a patient is taking olanzapine and smoking tobacco then increase the dose of the medication.
If quitting smoking = decrease dose of medication.
Enzyme Inducer example

This can lead to toxic levels.

Why do we use atypical antipsychotics first?
Less likely for EPS but cause more weight gain.
First generation antipsychotics risks
Higher incidemce of EPS
When a patient has increased risk for DTO/DTS when psychotic, you have to stabalize ASAP. What is one thing you can do to achieve this?
Use IM Invega (paliperidone), geodon, or Abilify.
Treatment for EPS symtoms
First line - Beta-blocker
Second line- Benztropine (anticholinergic)
Third line- clonazepam (use with care)
