First contact care, represents 80% of all care

Practitioner Care
Provides majority of care
Sustained relationship with patients in a
family/community setting
Can be hard to access for poor populations
Primary care is more of a privilege
Secondary
More specialized care, usually by primary provider referral
Usually for longer-term care
Two types: acute and subacute
Acute
intense, short-term treatment
for injury or quick illness
care accidents, trauma
Subacute
transitional care
Patient is stable, but may need
special monitoring, like burn wards
Tertiary
highly specialized, very sophisticated, very specific patient population
ICU and specialty hospitals

restorative and long-term care
Restorative
For patients who expect to
reach normative function
Usually done post-surgery or
after arrest of disease state
Rehab, therapy, home care
Long-term care
For patients who are not
expected to return to
normative care
Usually for chronic diseases,
disabilities, problems with
ADL
Common location is nursing
home
