SIMV
synchronized intermittent mandatory ventilation; won’t help above and beyond the set number; gives max
assist control
will help all breaths
CPAP
positive pressure only, doesn’t initiate any breaths
FiO2
fraction of inspired oxygen
tidal volume
ml of air based on body weight; normal 400-600; 10-12 ml/kg after surgery
SIGH
1 1/2 TV
rate
setting; record both ventilator and patient
PEEP
positive end expiratory pressure; helps keep alveoli open; increase peep for pt having a hard time oxygenating; increases FRC; negatively impacts cardiac output and venous return
pressure support
decreases work of breathing; only on spontaneous breaths; helps overcome the resistance of the ventilator circuit
exhaled volume
should be within 10% of tv
peak inspiratory pressure
pressure it takes to deliver tv; should be <40; if too low, something is disconnected
if the alarm sounds and you don’t know why…
bag them
complications of ventilation:
decreased CO: the lower the normal CO, the more significant it will be.
O2 toxicity: never need to over-oxygenate
barotrauma: volutrauma, atelectrauma, biotrauma
gastric distention
other complications:
infection, positive fluid balance, patient discomfort, psychosocial
weaning:
Mentate, Oxygenate, Ventilate, Expectorate
