defibrillation:
process by which current is passed through the heart to depolarize the cells and allow them to depolarize uniformly in an organized, coordinated contraction.
each minute we don’t defibrillate..
the chance of success decreases 7-10%.
defibrillate during exhalation because..
the lungs are deflated and it’s closer from the chest to the heart.
monophasic defibrillators
current from paddles through the patient; 1st shock is 200, 2nd is 200-300, 3rd is 360. never more than 360.
biphasic defibrillators
current from padres to patient back through the paddles; 150 at all times.
A/P defibrillation
under left clavicle and under left scapula.
cardioversion
should be timed so it’s on the QRS not the T; uses lower current; used if patient has a pulse and is stable.
therapy of choice for hemodynamically stable tachycardias
cardioversion.
pericardial thump
fist to chest; used when witnessed cardiac arrest and defibrillator isn’t available.
