Extra-mammary
What is cyclical mastalgia?
Associated with menstrual cycle
- Hormone related
Causes bilateral mastalgia
Is by far the most common
What is non-cyclical mastalgia?
Unrelated to menstrual cycle
Can be due to medication or more serious
- OCP
- Anti-depressants
- Anti-psychotics
What is extra-mammary mastalgia?
Chest wall pain
Shoulder pain
Tends to be muscular
Investigation for mastalgia?
Must be taken seriously
- Breast exam
- Scans depending on age
FNA if mass
PREGNANCY TEST!!
What scans are done for what ages?
USS <40
Mammogram >40
What are important questions to ask on history of mastalgia?
Skin changes
Discharge
Is it worse during cycle
LUMPS
1st line management of benign mastalgia?
NSAIDs
Bra fitting advice
2nd line for cyclical mastalgia?
Danazol
What is danazol?
Anti-gonadotrophin
3rd line?
Bromocriptine
What is bromocriptine?
Dopamine agonist
