Community interventions; walking groups, leisure classes, cooking classes
What it involved in Tier 2: primary care?
NHS healthy weight programmes, lifestyle advisor, community dietetic and drug therapy with local clinical guidance
What it involved in Tier 3: Specialist weight management?
Access to multidisciplinary team, i.e. psychological expertise, physiotherapy
What it involved in Tier 4: Specialist surgical service?
Bariatric surgery, gastric bands
What is used for non-surgical weight management in Tier 2?
Orlistat
What is the effect of orlistat?
Inhibits pancreatic and gastric lipase
What is the usual dose of orlistat given?
120mg 3 times daily

- Dietetic input (healthy balanced diet, with moderate energy deficit)
- Evidence based psychological model

When should bariatric sugery be considered?
Assessment of risk/benefit in patient who fulfil the following criteria:
• BMI > 35kg/m^2
• Presence of > 1 severe comorbidities which are expected to improve significantly with weight reduction
