Red flags for maltreatment on history
No history of traumatic event
MOI that isn’t compatible
Injury event not compatible with child’s development
Unexplained or unreasonable delay in seeking care
Clinical presentation red flags for maltreatment
Head injury with apnea
Intracranial injury and seizures
Intracranial injury and retinal hemorrhages
Radiographic red flags for maltreament
Subdural hemorrhages*
Rib fractures*
Skull fractures
Cerebral ischemia or edema
Classical metaphyseal fractures
How quickly should eye assessment by ophtho occur in children with intracranial hemorrhage?
Within 72 hours of injury
What ages is a skeletal survey done? When should a repeat occur?
Done in children <2 years old
Repeat after 14 days to assess for healing injuries that may not have been seen initially
What is the most common manifestation of child maltreatment
Bruising
Head injury is most common cause of death
Red flags for bruising in children
Non ambulatory
Ears, neck, face, buttock, torso (also chest, back, abdomen, genitalia)
Not on front of body
Large or numerous
Clustered or patterned
Does not fit with casual mechanism described
