What is a lymphoma? What are two subcategories of lymphomas? What are the subdivisions of NHL?
Neoplastic prolif. of lymphoid cells that forms a mass. May arise w/i node or extranodal; Non-Hodgkins and Hodgkins; Small B cell (Follicular, Mantle cell, Marginal Zone, Small Lymphocytic), Intermediate (Burkitt), Large (Diffuse Large B Cell)
What is Follicular lymphoma? Population? Mechanism? Tx? Complication?
Neoplastic Proliferation of CD20+ Small B Cells that form follicle-like nodules in LN. Late adulthood w/ painless lymphadenopathy; t(14:18) BCL2 gene on 18 translocates to very active Ig heavy chain gene on 14. Overexpression of BCL2 inhibits B cell apoptosis in the follicle; Low dose chemoTx/rituximab (anti-CD20 Ab); Progression to diffuse large B-cell lymphoma presents as enlarging LN
How do you distinguish follicular lymphoma from reactive follicular hyperplasia?
Disruption of normal LN architecture, Lack of tingible body macrophages in germinal center, Bcl2 expression in follicles, monoclonality
What is mantle cell lymphoma? Population? Mechanism?
Neoplastic prolif. of small B cells (CD20+) that expands the mantle zone; Late adulthood with painless lymphadenopathy; t(11;14) Cyclin D1 on 11 translocates to active Ig heavy chain on 14, overexpression of D1 results in G1/S transition
What is marginal zone lymphoma? Associations? What is a MALToma?
Neoplastic proliferation of CD20+ B cells that expands the marginal zone; Chronic inflamm states such as Hasimoto thyroiditis, Sjoegrens and H. pylori gastritis; Lymphoma of Mucosa associated lymphoid tissue
What is Burkitt Lymphoma? Classic presentation? Mechanism? Histologic findings
Neoplastic proliferation of intermediate-sized CD20+ B cells associated with EBV; Extranodal mass in child or young adult, African Type - associated with jaw, Sporadic - associated with abdomen; c-myc (8) translocates to active 14 (Ig heavy chain) which promotes cell growth; High mitotic index and starry-sky appearance
What is diffuse large b-cell lymphoma? Clinical significance? Mechanism? Population?
Neoplastic prolif of large CDC20+ B cells that grow diffusely in sheets; Most common NHL, clinically aggressive; Sporadic transformation of low-grade lymphoma; Presents in late adulthood as enlarging LN or extranodal mass
