Where does 75% of the lymph drainage in the breast?
to the axillary nodes which are lateral to the breast
Where is most of the lymph drainage that takes place in the breast?
it will drain to the closest node
all drainage will ultimately drain up to the apical nodes then to the subclavian trunk
Where does the inferior lateral part of the breast drain?
inferiorly to the pec. group nodes or
superiorly to central nodes
Where does the rest ~25% of the breast, which is the medial side drain?
internal mammary nodes (parasternal nodes)
What affects lymph drainage?
gravity, so body position influences lymph drainage
Where does cancer cells usually metastasize?
in the ducts of lymph
What are the artificial grouping of lymph drainage?
(low )level I - inferior to pec minor
(middle) level II - deep to pec minor
(upper or apical) level III - between pec minor and clavicle
What is the sentinel lymph node?
the first node that cancer has metastasized
What is a technique for detecting the sentinel node?
lymhoscintigraphy - radioactive tracer with a blue dye is injected into the breast. if found the blue die or radioactive signal will increase.
if found positive by pathology axillary dissection is required
What are the two types of breast implants?
subglandular
submuscular
What are the advantages of a subglandular breast implant?
quicker recovery but may lead tpo capsular contracture (scar tissue thickening)
makes mammograms harder to interpret
What are advantages of a sub muscular breast implant?
less prone to capsular contracture, easier for mammogram interpretation. harder to remove if problem longer more painful recovery
