How would you explain the procedure to pt and what would you check for prior to ax

A

We’ll be doing a lymph node assessment which involves me inspecting and palpating your lymph nodes as well as identifying structures around your neck area - is this okay with you?

Head - what are you ax

A

Inspecting:
Position - head is centred in the midline, erect and normocephalic (normal findings)
Size / Shape - medium round

Palpate - palpating skull for lumps, depressions or tenderness

Face - what are you ax and what do you palpate

A

Inspect for

Colour - uniform
Symmetry - aligned
Lesions/oedema/facial expressions
Involuntary movements - nil

Then palpate temporomandibular joint check for ROM tenderness of crepitus

Neck - what are you ax

A

Assessing colour, lesions, ROM, and any ENLARGMENT of accessory muscles.
No hypertrophy/atrophy
Symmetrical

Lymph nodes - state in which order you will palpate. Incl neck,

A

Occipital, post auricular, pre auricular, tonsillar, submanidular 1, submandibular 2, submental

Ask pt to face right
Identify trapezius/sternocleidomastioid muscle
x3 superficial cervical chain (in front of stern muscle)
x3 down posterior cervical chain (back of stern muscle)
^ on the THIRD one after palpating, leave fingers there and ask them to face you and look down for the deep superficial/posterior cervical chain.

then have pt return to central position and shrug shoulders, palpate (with both hands) Supraclavicular

A
I've assessed there to be no palpable head or neck lymph nodes however if I did I would be assessing for

What questions are key in asking the pt if u know ur going to do a lymph ax?

A

Any hx of sore throat, neck stiffness, decreased ROM, pain, swollen nodes, cough cold? flu like symptoms, ear infection - COVID SCREENING

Meningitis rt - headaches? light sensitivity? fevers? migraines?
