How would you explain the procedure to the client
So what we’ll be doing is a cardiac ax - this is broken into two parts. The first part will involve me inspecting and then palpating whilst identifying your landmarks on your chest, I will then listen to your apical pulse and other parts of your heart. This will require you to be laying down on your back with your chest exposed. Is this okay with you
How would you inspect the chest
Colour is uniform
Scars, lesions, oedema, temp(warm to touch)
Identify the landmarks your palpating towards apical pulse
Sternal notch Angle of louis 2nd RIGHT ICS 2nd LEFT ICS 3rd left ICS 4th left ICS 5th left ICS, midclavicular line (around nipple line)
How would you auscultate? State the heart valves - and the mnemonic
Auscultate the apical pulse for 60 seconds
Auscultate over the heart valves - aortic on right at 2nd ICS, pulmonic on the left 2nd ics, tricuspid on fourth near sternum and mitral at apical.
All pts take meds - Aortic, Pulmonic, Tricuspid and Mitral
What do the S1 and S2 sounds mean? and where are they loudest in the heart?
Which sound indicates blood being pushed into arterial system?
S1 - mitral and tricuspid valves closing together. always loudest at apex
S2 - aortic and pulmonic valves closing. loudest at the base
S2 represents blood being pushed into arterial system
What is normal findings of auscultation?
Normal expected heart sounds heard through all auscultated areas - no abnormal sounds or stenosis heard
What is aortic stenosis? What does it sound like?
Thickening and narrowing of the aortic valve. Sounds like a murmuring sound
The JVP tells you information of the ____ side of the heart
Right - predominantly R atrium pressure
Peripheral vascular assessment - the mnemonic is COWSCAMP - what does this stand for?
Colour Oedema Warmth Sensation CRT Appearance Movement Pulses
What are you looking at when inspecting in peripheral vasc part?
Scars, lesions, oedema, erythema(redness), ulcers, hair
Whats something important to look for when assessing peripheral vascular? In inspection?
Shiny legs - this can indicate PVD.
