Lumbar Interventions - MC Flashcards
Motor perfomance and function (Motor control/stabilization) encompasses….
strength, endurance and coordination generally of the “core”
MP and function has been shown in the research to be successful with the Tx of LBP by:
- Decrease pain
- Improve function and activity tolerance
- Decrease work-related disability
- Prevent recurrence of LBP
- Improve function Post microdiscectomy
IAP - Stability of Joints
Box/Can Concept
* Diaphram is the roof
* Mutifidus and Transverse Abdominis are walls
* Pelvic floor is the floor
When generating pressure within the abdominal cavity it supports the spine
How does the pressure in the abdomen change with breathing?
Breath in: Diaphram contracts down and expands. AC, PC, PF come out.
Breath out: Diaphram relaxes, AC comes in and PC and PF come up and contract
What muscles tend to be tight and which are inhibited?
- Postural Examples: QL, erector spinae, hamstrings, hip flexors, TFL, piriformis, rectus femoris, upper trap, scalenes, biceps
- Phasic muscles: abdominals, glut med/max, quads, soleus
What are some qualifications for a stabilization exercise to be effective?
- Age less than 40
- Positive prone instability
- Prescence of aberrant motion with AROM (Gower;s sign, instability catch, reversed lumbopelvic rhythm
- Greater than 91 degrees on SLR
- Hypermobile patients: so mobile they rely on ligaments and bones rather than muscel resulting in ligaments getting stretched out over time
One session of ____ can lead to less LBP
Motor Control
Directional Preference - Lateral Shift Irriatibility and Exercise
- Low to moderate: Standing with shoulder into wall and slowly shuffling to full stand
- High: Prone or Supine
Directional Preference Extension Irritability Exercise
High Irritability
* Prone with towl under hips
* Prone press ups
* Standing extension
* Standing extensions with pressure through spinal level for extension
Directional Preference Flexion Irritability Exercises
High
* Posterior Pelvic Tilts
* Single Knee to Chest
* Double knee to Chest
* Hamstring stretch in supine
* Partial abdominal curl-ups
Low
What do neurodynamic exercises do?
- Increase blood flow and axoplasmic flow and decrease mechanosensitivity
How do specifically examine the Femoral Nerve?
- Knee flexion in prone with cervical flexion, relieved with cervical extension
What is a better alternative word for exercise?
movement
Traction
- No benefits to non-radicular pain
- Short term benefits of intermittment traction for patient with radicular Sx’s with: peripherlization with extension and/or + Well leg raise (SLR and get Sx on opposite leg)
Aquatic Therapy and BW
- Unloads the spine but is also active (therefore better than traction)
