Distribution

  • Always rectum involved unless on anti-inflammatory enemas
  • No skin lesions
  • “Backwash” ileitis possible if ileocaecal junction involved.

Micro

  • Mucosal inflammation
  • Goblet cell depletion
  • Crypt of Lieberkuhn distortion
  • Vascular congestion
  • Branching crypts

Extra-colonic manifestations of UC

A

Respond to colectomy:

  • Peripheral arthropathy (present in 20%)
  • Erythema nodosum (present in 10-20%)
  • Iritis

Do not respond to colectomy:

  • Pyoderma gangrenosum (present in 1-10%)
  • Axial athropathy (present in 5%)
  • PSC (present in 5%)
  • Uveitis
  • Episcleritis (most common eye disease)
