- Always rectum involved unless on anti-inflammatory enemas
- No skin lesions
- “Backwash” ileitis possible if ileocaecal junction involved.
Micro
- Mucosal inflammation
- Goblet cell depletion
- Crypt of Lieberkuhn distortion
- Vascular congestion
- Branching crypts

Extra-colonic manifestations of UC
A
Respond to colectomy:
- Peripheral arthropathy (present in 20%)
- Erythema nodosum (present in 10-20%)
- Iritis
Do not respond to colectomy:
- Pyoderma gangrenosum (present in 1-10%)
- Axial athropathy (present in 5%)
- PSC (present in 5%)
- Uveitis
- Episcleritis (most common eye disease)
