What is the pathogenesis of liver injury?
HSC’s in the space of disse become activated, turn into myofibroblasts, which then deposit collagen and causes the fibrosis
What is the clinical presentation of cirrhosis?
Those of portal hypertension
Presentation of portal hypertension?
Ascites, splenomegaly, GI bleeding
What is the difference between compensated and decompensated liver cirrhosis?
Compensated shows no clinical manifestations of decompensation
What are complications of cirrhosis?
HCC, hepatic decompensation, and portal hypertension
What are the manifestations of HCC?
Right hypochondrium pain, enlarged liver
What is acute-on-chronic liver failure?
A syndrome of acute hepatic decompensation
What is the psychophysiolgy of ACLF?
Systemic inflammation
What does ACLF present as?
The worsening of cirrhosis related complications
What is the gold standard for liver cirrhosis diagnosis?
Liver biopsy
What is liver elastography?
Mechanical waves are propagated through the liver, they measure the stiffness of the liver, value of >12.5kPa is suggestive of cirrhosis
What is the aim of treating cirrhosis?
Preventing progression
What is the aim of treating compensated cirrhosis?
Preventing bleeding from esophageal varices, via endoscopy and NSBBs’ (non selective beta blockers)
What is Hyperdynamic circulatory syndrome?
Found in cirrhosis and portal hypertension patients, it’s an increased cardiac output with a rapid heart rate with a systolic murmur
What is the main cause of the hyper dynamic circulatory syndrome?
Systemic vasodilation
