Linear IgA bullous dermatosis/chronic bullous disease of childhood Flashcards
1
Q
Ages affected by linear IgA bullous dermatosis/chronic bullous disease of childhood?
A
- elderly ( >60) and preschool-aged children (average age 4)
2
Q
Adult onset linear IgA bullous dermatosis is caused by_____
A
drug induced
3
Q
Most common drug causing Adult onset linear IgA bullous dermatosis?
A
Vancomycin
4
Q
Pathogenesis of LABD/CBDC (linear IgA dermatosis)
A
- IgA autoantibodies directed against two antigens derived from BPAG2:
- LAD-1
- LABD97
5
Q
What are the antigens targeted by IgA autoantibodies in LABD/CBDC?
A
- LAD-1
- LABD97
6
Q
Describe clinical presentation of LABD/CBDC.
A
- tense vesicles/bullae and urticarial plaques in annular, polycyclic, or herptiform (crown of jewels) arrangement
7
Q
Most common site affected in LABD/CBDC?
A
- flexures of lower trunk/thigh/groin/buttocks and face in kids
8
Q
What can you see at peripheral edge of lesiosn of LABD/CBDC?
A
- vesiculopustules at expanding edge of plaques
9
Q
How can you distinguish LABD/CBDC from DH?
A
- need DIF (histopath cannot distinguish)
10
Q
Histopath of LABD/CBDC?
A
- neutrophils diffusely lined up along BMZ (early lesions)
- subepidermal blister w/ neutrophils inside cavity (fully developed lesions
11
Q
DIF of LABD/CBDC will show?
A
- linear IgA along BMZ
12
Q
Treatment of choice for LABD/CBDC?
A
- Dapsone leads to rapid response in 72 hours (remember that this is histologically similar to DH and tx is the same)
