Therapeutic Effects of lidocaine
Suppress ventricular ectopic activity
Decrease excitability of heart muscle & cardiac conduction system
Indications for lidocaine
Suppress PVCs
Prevent recurrence of VFib after electric conversion
Treat VTach
Suppress reflex rise in ICP during intubation
Contraindications of lidocaine
Allergy to lidocaine or local anesthetics
2nd or 3rd degree heart block
Sinus bradycardia or sinus arrest
Idioventricular rhythm
Side Effects of lidocaine
Fall in cardiac output & BP due to decrease force of cardiac contractions & decrease peripheral resistance
Numbness, drowsiness, confusion if given in high doses
Seizures in pt in heart failure
Adult Dose of lidocaine
1.5mg/kg IVP then 50mg bolus q20min
1mg/kg IVP prior to intubation of head injured pt
Pediatric Dose of lidocaine
VFib: 1mg/kg IVP, IO or ET
