what are provisions for practice after graduation but before receiving license?
90 days or until candidate fails the exam, whichever comes first.

what is required for renewal?
CEU
24 CE hours from a variety of categories,
must attest to CEU requirement,
must attest to disciplinary status.
License expires on the 1st of May of the next even year following date of original licensure. Before this date the department should mail a renewal application, evidence of 24 hours of CE, and renewal fee.
The department may reinstate any person who fails to renew their license in time by paying an additional fee.
Any PT who is practicing during that time while their license has lapsed is subject to penalties.
Direct Access; or from whom can referrals be accepted?
medical doctor, dentists, podiatrists, chiropractors, osteopaths

- PT must disclose their scope and limitations of practice and obtain consent in writing.
- Must refer the patient (where relevant) within 90 days after initial treatment (unless treatment has ended by then).
- PT must have at least 1 year of clinical experience
supervision of PTAs?
PT shall at all times be responsible for the supervision of the PTA.
Supervision shall include but not be limited to the following:
verbal or written contact with the PTA; and maintenance of the following written documentation of:
- Necessary instruction, on-site supervision of the PTA’s performance,
- review of the PTA’s documentation,
- a reassessment and appropriate updates of the patient’s program and goals.
All of the above should occur at least every 4-6 patient visits but no less than once a month.
Supervision of supportive personnel
The PT shall at all times be responsible for the supervision of supportive personnel and students.
Supervision shall include but not be limited to the following:
contact with supportive personnel and maintenance of the following written documentation:
- Necessary instruction and documented competence
- line of sight supervision of the treatment performed by the supportive personnel
- and review of the supportive personnel’s documentation on a daily basis.
Under what circumstances can a license be revoked?
Includes but is not limited to:
fraud or deceit in procuring or attempting to procure a license or in the practice of PT;
habitually intemperate or is addicted to the use of habit forming drugs; mentally and/or professionally incompetent;
providing services to a person who is making a claim as a result of a personal injury,
charges or collects from the person any amount in excess of the reimbursement;
conviction;
abandonment of a patient; promotion of the sale of drugs, devices,
goods or services for a patient in a manner as to exploit the patient;
making/filing a false report in the practice of PT; repeated failure to file or record, impede or obstruct a file; etc.
