Gamma-Glutamyl Transpeptidase (GGT)
- Determines source of Alkaline Phosphatase elevation
- Sometimes used as marker of alcohol consumption
What indicates liver inflammation/hepatocellular damage?
↑ALT & ↑AST
What is the pattern for cholestasis?
↑Alk Phos, ↑Direct, ↓urine Urobilinogen
What indicates reduced liver fxn?
↑PT/INR, ↓Albumin
What is the common cause of cholestasis or obstructive jaundice?
Choledocholithiasis
What does choledocholithiasis present w/?
- Pain – biliary colic
- Jaundice
- Clay-colored stools
- Cola colored urine
What does Gilbert Syndrome cause in lab test?
Elevation in indirect bilirubin
What is Gilbert Syndrome?
- Benign condition
- Usually only seen in homozygotes
What causes Gilbert Syndrome?
Defect in the promotor of the gene that encodes the enzyme uridine diphosphoglucuronate-glucuronosyltransferase 1A1 (UGT1A1), which is responsible for the conjugation of bilirubin w/ glucuronic acid
What is Alkaline Phosphate derived from?
- Liver
- Bone
- Therefore, must follow up w/ GGT to determine source
What does elevated GGT indicate?
Source = liver
If GGT is normal, what does this indicate?
Source = Bone
