Leukopenia and leukocytosis Flashcards
treatment of leukopenia
GM-CSF or G-CSF to boost granulocyte production
Causes of leukopenia
Drug toxicity
Severe infection
Causes of lymphopenia
immunodeficiency (digeorge)
high cortisol state
autoimmune destruction
whole body radiation
Causes of leukocytosis
Bacterial infection or tissue necrosis
high cortisol state-impairs leukocyte adhesion leading to release of marginated pool
What indicates that neutrophils are immature (left shift)?
decreased Fc receptors (CD16)
Cause of monocytosis
chronic inflammatory states (autoimmune and infectious) or malignancy
Causes of eosinophila
allergic reactions
parasitic infections
hodgkins lympoma via release of IL5
causes of basophila
chronic myeloid leukemia
causes of lymphocytic leukocytosis
Viral infections
Bortedella pertussis -produces lymphocyte-promoting factor which blocks lymphocytes from leaving blood
two VIRAL causes of lymphocytic leukocytosis ?
EBV and infectious mononucleiosis
Where does EBV infect?
B cells
oropharynx
liver
Pathophys of EBV
CD8 cell response causes generalized lymphadenopathy due to t-cell hyperplasia in lymph node paracortex
-this also leads to splenomegaly
Which test is used to identify EBV?
Monospot test- detects igM antibodies that cross react with sheep or horse RBCs (heterophile antibodies)
What does a negative monospot test suggest?
CMV may be cause
Definitive test for EBV?
serologic testing for EBV viral capsid antigen
