- Developmental level
Include information that identifies the client name, address, gender, birth date, Phil heath number, medical record number, etc.
Biographic data
Focused on questions related to the client’s personal history, from the earliest beginnings to the present
Personal health history
Identify health problems that seem to run in families and that are genetically based.
Family health history
This is a very important section of the health history because, it deals with the clients human responses
Lifestyle and health practices profile
Know how the client sees his usual pattern of daily activities
Description of Typical day
Recall what consists of average 24-hour intake, emphasis on what food are eaten, what amounts, who buys and prepare food…
