What is Lemierre syndrome?
Thrombophlebitis of the jugular veins with distant septic mets in the setting of oropharyngeal infection
Pathogens most commonly responsible for Lemierre?
- Fusobacterium necrophorum
- 30% polymicrobial
- Anaerobic streps / G-ve anaerobes
CFx Lemierre syndrome?
- Trismus
- pain behind angle of jaw
- neck swelling
US Fx Lemierre syndrome?
Thrombus within jugular vein / other neck or facial veins
Fx to identify on CECT Lemierre syndrome?
- Intraluminal filling defect JV
- Sites of septic emboli (usu pulmonary)
- Site of primary infection
Ages commonly affected?
Young adults or adolescent
Common cause of sepsis in Lemierre?
- Tonsillar or peritonsillar abscess and
- thrombophlebitis of the tonsillar or peritonsillar veins (spreads to IJV/facial vein)
Causes of IJV thrombosis?
- CVC
- IVDU
- Hypercoaguability
- Infection
- Atherosclerosis
CFx IJV thrombosis?
- Swelling/tenderness along SCM and angle of mandible
- +/- cellulitis, trismus, dysphagia
Most common sites of septic mets?
Lungs and joints
can go to liver, soft tissue, bone
Rx lemierre?
Fusobacterium usu respond to:
-Penicillin G
-Anerobic cover (metronidazole, clindamycin, cefoxitin, chloramphenicol)
Prolonged Rx (as endovascular)
