what occurs with increasing levels of neutrophils?
- packed with elastase and collagenase
- can digest through the airway tissue
- results in scar collagen being left in airways
- epithelial fibrosis and airway limitation
what are the advantages of research with lung transplants?
- build lung function
- samples easily taken
how does social function change years after a lung transplant?
- lower social function
- equivalent to people suffering chronic illnesses
what was found in hydroxychloroquine?
- has effect on epithelial cells / cell lines
what is total lymphoid irradiation?
- type of immunosuppression for patients undergoing transplantation that have refractory allograft rejection during standard immunosuppressive therapy
what is most injury after lung transplant associated with?
- low immunity
what is fundoplication?
- tightens the lower oesophageal sphincter by wrapping round
- causes less reflux back up oesophagus
what occurs if fundoplication wrapped round too tight?
- food wont be able to move down oesophagus
did fundoplication improve freedom from BOS?
- yes
- took longer for patients to gain BOS after the surgery but lots still gained
what is the issue with this treatment?
- if cant tolerate anaesthetic
what is azithromycin ?
- used as anti-malarial therapy
- already had safety benefits / experiments
how does baseline change with administration of azithromycin?
- fluctuating values
- people still deteriorate but can gain lung function
when are patients classed as responders?
- when improvement was 10% or higher
- compared to baseline values at start of azithromycin therapy
what were the results from induction therapy with azithromycin in BOS?
- not much effect on overall survival
- helps but means on long term antibiotics
what was the effect of azithromycin in CF?
- 12/14 infected with nontuberculous mycobacteria (NTM)
- proves link between NTM and CF
